New ICER Report Takes Hard Look at Spravato

The Institute for Clinical and Economic Review (ICER) released a statement on June 19th questioning the efficacy and cost effectiveness of treating resistant depression with esketamine (Spravato). The “report-at-a-glance”, linked *here*, summarizes the *full report*. ICER believes a reduction in price of 25-52% would be needed to improve cost-effectiveness. Spravato’s current list price of $32,400 per year exceeds the standard cost effective ratio that ICER has put in place.

When compared to the price of ketamine via IV infusion, Spravato in the first year reached a total of $36,500 while ketamine came in at $3,600. For persons seeking life-saving treatment, this barrier to entry poses a significant hurdle without proof of superiority to existing therapies (i.e. IV ketamine, TMS, ECT). While medications like Spravato are the future of depression treatment due to shortened response times and ease of use, we at Sunbelt Wellness Institute would like to see cost addressed by Janssen Pharmaceuticals.

Lastly, the policy recommendations highlighted in this report point towards changes at the level of the manufacturer. A comparison between esketamine and current treatments such as off-label ketamine via IV, transcranial magnetic stimulation (TMS), and electroconvulsive therapy (ECT) would allow real-world, long-term assessment of effectiveness. This last point means the most to patients who may have had prior success or failures with these highlighted therapies.
